The Chronic Conditions Alliance was formed after the completion of the Health Partners Self Management Project in 2004. The Alliance met bi-monthly at SHOUT and was a network of community organisations working together to improve the quality of life for people living with a chronic condition.

The last meeting held by the Chronic Conditions Alliance was in April 2006. At that stage the Alliance had developed a list of priority issues/concerns, a web presence on the ACT Division of General practices web site and had invited a number of guest speakers to meeting to provide information on specific topics.

On 26 June 2009 organisations who were involved in the Alliance previously were invited to meet again to gauge interest in reforming the Chronic Conditions Alliance, in what format, whether identified issues have changed and how to proceed.

One of the outcomes of the meeting was that the Alliance should have it's own website, work has commenced and it can be found at www.actcca.org.au
